Initial evaluations and follow ups are 60 minutes of one on one care with physical therapist Emily Thoes. Movement assessments are 30 minutes.

Initial evaluations are $200 and follow up sessions are $125. Fees are due at time of service. Cash, card, HSA/FSA are accepted. I am out of network with all insurance plans, but can provide you with a superbill to submit to your insurance for reimbursement if you would like to use your out of network benefits.

I am an out of network provider for all insurance plans. This allows me to provide personalized care without limitations or complexities due to insurance-based rules. Quite simply, it allows you to get more efficient and personal care without needing to get treatments pre-authorized or approved by your insurance.

Check Your Insurance Plan: Review your insurance policy to understand coverage for out-of-network services, including any deductibles, copayments, and reimbursement rates. Your policy will need to include out of network coverage in order to be eligible for reimbursement.
Attend Your Appointment: Attend your physical therapy appointment and pay for your session at the time of service.
Request Your Superbill: A “superbill” is a detailed receipt from Training Health. You will use this to submit for reimbursement from your insurance company.
Submit the Claim: Use your insurance company’s online portal to submit your claim along with any other requested information (such as group number or member ID). After submission, monitor the status of your claim. You can call your insurance provider to confirm receipt and inquire about the timeline for processing.
Review the Explanation of Benefits (EOB): Once processed, your insurance company will send an EOB detailing what was covered and how much you’ll be reimbursed.

You do not need a doctor’s referral to schedule your initial evaluation. Due to Texas state law, you will need a referral within 10 business days of starting physical therapy. This referral can be from your primary care physician, orthopedic doctor, OB/gyn physician, chiropractor, nurse practitioner, or dentist. I can help you find a provider for referral to PT if needed.
Pregnant patients will need written clearance and referral from their OB/Gyn or midwife in order to participate in pelvic floor physical therapy. Postpartum patients will need written clearance by the OB/Gyn in order to start pelvic floor physical therapy. This is usually at the 6 week postpartum appointment with OB/Gyn.

Orthopedic PT appointments take place at Diesel Fitness - 2901 Cityplace W Blvd #100, Dallas, TX 75204. Pelvic floor PT appointments take place at the patient’s home. Pelvic floor patients may also schedule sessions at Diesel Fitness when appropriate.

Complimentary street parking is available on Cityplace W Blvd, conveniently located in front of the gym. Additionally, you can park for free in designated Diesel Fitness spots within the parking garage, accessible from either Noble Ave or Oak Grove Ave. Upon entering the gym, please remember to scan the QR code to register your vehicle as a guest. Please give at least 24 hours notice for cancellations. This allows me to offer the appointment slot to other clients who may be waiting for services.
If an appointment is canceled with less than 24 hours notice, a late cancellation fee of $75 will be applied. This fee helps cover the costs associated with the missed appointment and ensures I can continue providing high-quality care to all our clients.
